Boulevard Labs, Inc. (“Boulevard”) provides appointment booking, scheduling, and customer engagement platforms (the “Services”) to self-care businesses with a Boulevard account (each, a “Boulevard Customer”). When consumers of our Boulevard Customers (the “Client(s),” “you”) book or manage appointments with a Boulevard Customer using the Services, including through Boulevard’s AI‑powered features or voice receptionist, Boulevard acts as a service provider processing information on behalf of that Boulevard Customer.
This Privacy Notice covers only how Boulevard collects and uses your personal information in its capacity as a service provider. It does not cover how Boulevard Customers use your personal information; that use is governed by the Customer's own privacy practices and policies. We encourage you to review your Boulevard Data Privacy Addendum to understand how they handle your information.
For purposes of this Privacy Notice, “personal information” means any information relating to an identified or identifiable individual , and includes “personal information,” “personal data” or similar terms as defined in applicable law. Personal information does not include information that Boulevard or the Boulevard Customer may de-identify or aggregate so that it will no longer be considered personal information.
This Privacy Notice is directed to Clients. If you are a Boulevard Customer, please review the Privacy provisions in our Main Services Agreement and Data Privacy Addendum.
1) Our Data Collection Practices
When using the Boulevard Services to make or manage an appointment or make a payment, Boulevard may collect the following categories of personal information from Clients on behalf of Boulevard Customers:
Identifying information, such as name and contact details (email address, phone number).
Appointment and booking information, such as appointment date, time, location, and service type, and notes or preferences entered during booking, which may include health-related information, where you book with a Boulevard Customer offering healthcare or wellness services.
Communications data, such as messages exchanged through the platform and interactions with automated or AI-powered receptionists. We may record calls to our AI voice receptionist for quality assurance, training, and analytics purposes.
Payment-related information, including limited payment details necessary to facilitate transactions.
Technical and usage information, such as device, browser, IP address, and interaction logs related to booking and scheduling functionality.
Booking overlay and integration data, such as your interactions with Boulevard's booking overlay embedded on a Boulevard Customer's website (the "Booking Overlay"), including tracking data collected through cookies, pixels, and similar technologies, as further described in Section II of this Privacy Notice.
2) How We Use Personal Information
Boulevard’s contract with Boulevard Customers determines how Boulevard or the Boulevard Customer may use the personal information collected through the Services. As examples, Boulevard and Boulevard Customers may use the personal information collected through the Services to:
Facilitate appointment booking and scheduling;
Deliver confirmations, reminders, and updates;
Enable AI‑assisted booking, customer support, and business communications;
Support Boulevard Customer operations and provide Customers with analytics, reporting, and insights about their business performance;
Maintain, analyze, secure, and improve the Services;
Analyze how the Services are accessed and used, measure performance and effectiveness, and generate aggregate or de-identified analytics and reports;
Conduct internal research and development to improve existing features and develop new products and services;
At the direction of the Boulevard Customer, enable third-party technologies that collect information about your activity using the Services to display personalized advertising to you across the internet;
Monitor, protect, and maintain the security and integrity of our Services and our business, such as protecting against and preventing fraud, unauthorized transactions, claims and other liabilities;
Comply with applicable laws and regulations and respond to lawful requests and communications from law enforcement and other government officials;
Carry out sales and business transactions;
Protect our rights, privacy, safety, property and/or those of Boulevard Customers, Clients, or others;
Fulfill any other purpose for which you provide personal Information or as explained to you at the point of information collection; and
For any other purpose disclosed by the Boulevard Customer in its separate privacy notice.
3) How We Disclose Personal Information
We make the personal information that we collect available to Boulevard Customers or other third parties at the Boulevard Customers’ direction. Please consult the Boulevard Customer’s privacy notice for additional information. Please note that once personal information is made available to a Boulevard Customer, Boulevard does not control and is not responsible for how that Boulevard Customer uses, shares, or otherwise processes such personal information. We may also disclose personal information to third-party service providers that assist us in offering the Services, including by providing business, professional, or technical support services.
In certain circumstances, Boulevard may be required to disclose personal information as follows:
Governmental, Regulatory, and Public Authorities. We may disclose personal information to government and law enforcement authorities, in each case to comply with legal or regulatory obligations or requests.
Relevant Third Parties as Part of a Corporate Transaction. In the event of a reorganization, merger, sale, joint venture, assignment, transfer, or other disposition of all or any portion of our business, assets, or stock (including in connection with a bankruptcy or similar proceeding), we may disclose or transfer personal information to certain third parties, such as the acquiring entity and its advisers.
Other Third Parties. We may disclose personal information to other third parties at your direction or with your consent. Additionally, we will disclose your personal information as we believe necessary or appropriate to: (a) comply with applicable law; (b) enforce our terms and conditions; (c) protect our operations; (d) protect our rights, privacy, safety, or property, and/or those of others; and (e) allow us to pursue available remedies or limit damages that we may sustain.
We may disclose your personal information for other reasons that we will describe at the time of information collection or prior to disclosing your information.
4) How we collect personal information
When you interact with the booking interface embedded on a Boulevard Customer’s website (the “Booking Overlay”) or our Services, Boulevard, the Boulevard Customer, or any service providers or third parties operating on behalf of Boulevard or the Boulevard Customer may collect personal information from you directly or through automated means.
For example, we collect personal information that you voluntarily enter into the Booking Overlay forms, including details about your appointment request and information that identifies you, such as your name, email address, telephone number, address, or other contact details.
Additionally, we and our analytics, fraud-prevention, or security partners and service providers as well as authorized partners of our Boulevard Customers may automatically collect personal information using cookies, web beacons, pixel tags, software development kits, and similar technologies to automatically collect certain information about your device and your interactions with the Services. Information collected may include:
Device and browser details, such as IP address, browser type, operating system, and device identifiers;
Usage information, such as pages visited, features used, time spent, and interaction data within the Booking Overlay;
Referral data, such as the webpage that directed you to the Booking Overlay; and
Session information, such as form inputs and steps taken during a booking session.

5) Data Retention
Boulevard will retain personal information as required in accordance with its agreement with the Boulevard Customer and applicable law. Please consult the Customer’s privacy policy for additional information on data retention practices.
6) Your Rights
Clients may have certain privacy rights with respect to their personal information collected via the Services, including but not limited to:
The right to know which categories of personal information have been collected about the Client, and the right to access such data;
The right to correct the data maintained about the Client;
The right to delete the data maintained about the Client; or
The right to opt out of sales or sharing of personal information or the processing of personal information for targeted advertising.
